How do we report the problem when we find an error is an important criteria.
In general following information should be covered in Problem Reporting:-
1. Identification of report.....say by Sr No.
2. Circumstances of the error
3.Who is authorising a fix?
4.Who needs a fix and when?
5. Change report upon fixing the error

Change report includes a problem report and adds the following information--
a. Was the error confirmed as valid?
b. What was the cause of the error ?
c. What changes were made?
d. What tests were run to confirm that?

NOTE: Members are invited to produce practical Problem Report model they have experienced.
